The integration mistakes
Three failure modes we have seen often enough to recognise from the discovery call.
Mistake 1: voice without writeback
Some AI vendors will deploy "AI phone receptionists" without integrating into the practice management system. The patient feels booked. The clinic does not know. Patients show up, the clinic is not expecting them, and chaos follows.
The fix: synchronous writeback. The AI does not say "you're booked" until the booking is verified in Dentrix or whichever system.
Mistake 2: insurance over-promise
The AI confidently quotes coverage. The patient arrives. Their plan does not actually cover what was quoted. The clinic eats the cost or has an uncomfortable conversation.
The fix: the AI never quotes coverage. It says "we accept your plan, the specific coverage will be verified during your visit." For real-time eligibility checks, integrate the payer API and surface a structured result, not a guess.
Mistake 3: emergency routing left to chance
The AI handles a call from a patient with severe post-op bleeding the way it handles a booking call. The patient gets a polite "we can see you next Tuesday."
The fix: emergency keywords trigger a hot transfer to the on-call dentist with structured context. We log every emergency call separately and review them weekly.
Multi-location DSOs
A different shape of the problem.
A single clinic deploying an AI receptionist gets the patient-experience and revenue win directly. A DSO running 15 clinics has a different challenge: the experience needs to be consistent across locations, the routing needs to send the patient to the closest available appointment, and the reporting needs to roll up at the parent level.
What works at DSO scale:
- A single AI infrastructure layer across all clinics, configured per location.
- Centralised reporting on call volume, booking conversion, missed-call rate, and patient lifetime value by clinic.
- Per-clinic overrides for knowledge (different services, different insurances, different hours).

What we will not deploy
Two things some vendors will sell that we will not.
1. Diagnosis or treatment advice
The AI does not give clinical advice. Not "do I need a root canal," not "is this normal post-op," not "should I be worried about this pain." Every clinical question routes to a human, fast.
The reason is legal and ethical. The reason is also that current AI systems are wrong often enough that the floor matters more than the ceiling.
2. Insurance pre-authorisation decisions
The AI collects insurance info. It does not make pre-auth decisions or commit the clinic to coverage. That stays with the billing coordinator.
What rollouts actually look like
Five business days from kickoff to live for a single-location clinic. Two to three weeks for a small DSO. Here is the breakdown:
- Day 1: 90-minute knowledge intake with the clinic owner and the front desk. We capture hours, services, insurances, common questions, emergency rules, and tone.
- Day 2: Voice tuning (we re-record 3-5 times before approval) and integration setup with the practice management system.
- Day 3: Internal testing across ~80 call scenarios. We listen to every test call.
- Day 4: Live on overflow only. The AI takes calls when the front desk is busy or after hours. Every call is reviewed.
- Day 5: Full cutover. The AI is the main line. Front desk pivots to in-person.
